Remission of Alopecia Universalis with Dupilumab in a Patient with Severe Atopic Dermatitis

A recent report describes a case of a 49-year-old woman who presented with alopecia universalis since 2011 and received treatment with oral betamethasone 0.5 mg per day without clinical benefit. She also reported a history of early-onset severe atopic dermatitis, allergic rhinitis, asthma and polysensitization, for which she had received treatment with oral and topical corticosteroids and ciclosporin with poor clinical response and a chronic-relapsing course. 

 

In January 2019, she received dupilumab therapy with a 600 mg induction dose followed by a 300 mg injection every 2 weeks. Her disease severity scores were 24 for the Eczema Area and Severity Index (EASI), 10/10 for the Numerical Rating Scale of itch (NRSi) and 100 for the Severity Alopecia Tool (SALT). 

 

In May 2020, she revealed a partial regrowth of terminal hairs and body hairs with an EASI score of 0, NRSi 1/10 and SALT score of 70/100. In her last follow-up, in December 2020, she showed marked improvement in the clinical response, achieving a SALT score of 10/100.
